Campral (acamprosate calcium) is indicated as a therapy to maintain abstinence in alcohol dependent patients. It should be combined with counselling. Campral does not constitute treatment for the withdrawal period.
Campral comes as a 333-mg enteric-coated tablet with weight mediated dosages of 2 tablets tds for adults over 60 kgs. and 2 tablets mane, 1 midday and 1 nocte for adults 60 kgs or less for at least one year. The dosage should be maintained if the patient relapses.
Controlled studies show that relapse rates in alcoholism are lower in acamprosate-treated patients than in patients treated with placebos. At one year, abstinence rates are about 19% in acamprosate-treated patients versus 8-9% in placebo treated subjects. At two years, the rates of abstinence are apparently 12% and 5% respectively. However, complete abstinence may not be the only goal to aim for with some patients.
Sass 1 found that 39% of acamprosate patients remained abstinent compared with 17% in their control group. In a double-blind multicentre study 2 with 569 patients, acamprosate 1.3 gram/day was more effective than placebo in maintaining alcohol abstinence for three months as assessed by plasma gamma-glutymyl transpeptidase. Whilst there was a significant decrease in both groups it was 32% lower in the acamprosate group than in the placebo group on day 60 and day 90.
Acamprosate 2g/day or 1.3g/day for 12 months was more effective than placebo in maintaining alcohol abstinence in another double-blind study 3 with 538 patients. Patients were assessed every three months and received supportive psychotherapy, antidepressants anxiolytics and continued any maintenance medication. Acamprosate was better than placebo in maintaining continuous abstinence at 12 months: 18.1 % of the 1.3 g/day group, 19.1% of the 2 g/day group, and 11.3% of the placebo group had been continuously abstinent. During the treatment period, the average number of days of continuous abstinence was significantly higher in the high-dose acamprosate group than in the placebo group with the average time to first relapse being 135 days, 153 days and 102 days respectively. At 12 months 28%, 35% and 19% respectively of patients were abstinent.
Alcohol consumption by patients who were abstinent at the 12-month follow up but who drank during the 12-month period was less frequent in the acamprosate groups. However, the abstinence rate was no longer significant 6 months after the treatment period.
Acamprosate was again found to be more effective than placebo in maintaining abstinence from alcohol in a multicentre placebo-controlled study 4 of 179 patients who completed the 12 months of treatment. At the end of the 360-day treatment period 18.3% of acamprosate and 7.1% of placebo patients had been continuously abstinent. The mean cumulative abstinence duration was also significantly greater in the acamprosate group than in the placebo group (138 Vs 119 days respectively). Of the 148 patients who completed the 12-month follow-up after the treatment period, 11.9% and 4.9% respectively were continuously abstinent at the end of the two-year study. The cumulative abstinence duration was significant for the entire two year study (acamprosate group = 230.8 days and placebo group = 183 days).
Poldrugo 5 also found that acamprosate treatment for alcoholism resulted in longer time to first relapse and a greater cumulative duration of abstinence than did the placebo treatment. 246 free-living were given a six months supply of either acamprosate or placebo and they reported to alcoholism treatment centres for assessment after 1,3,6,9, and 12 months. A significantly greater proportion of acamprosate subjects was abstinent at 1,3, and 6 months (the treatment period). The mean time to first relapse was 150 days for the acamprosate group and 61 days for the placebo group. During the 6 months' follow-up after cessation of treatment, the abstinence level declined in both groups but remained significantly higher for the acamprosate group at 1 year. The only side-effect detected in acamprosate treatment was diarrhoea, which occurred in about 20% of the acamprosate and in 12% of the placebo group for the first 7 days. 6
Mann 7 concurs with the above studies and concludes that although existing treatments (e.g. AA, therapeutic communities, counselling, etc) are often highly efficient, they reach few of the patients in need. GPs may come to play an especially important role in future therapy programs, provided they have the tools necessary for assessing and diagnosing (eg. CAGE - see GPSpeak, December 99 and motivational interviewing - see GPSpeak, October 99) and treating alcohol dependent patients. One such tool could be this drug that appears to reduce craving for alcohol and enhances abstinence.
As of November 1, 1999, Campral became available in Australia as a PBS listed item (authority required). The PBS listing states: "Authority required; for use within a comprehensive treatment program for alcohol dependence with the goal of maintaining abstinence." Maximum quantity 180 tablets with 1 repeat. PBS price $171.59.
Acamprosate appears to have a safe profile with two contraindications for use being renal dysfunction and severe liver failure. There is no experience of its use in pregnancy and as it passes into breast milk, it is not recommended for use in either of these situations. Naltrexone is likely to be more acceptable to patients with its once a day dosing, whereas acamprosate is based on weight and thrice daily dosing.

My freind is a self confessed achohlic she is atteding aa and does not find this threatining, although she does not what to give up drinking she does not beleive the campral has had so much a change to her life ( but I beleive it has ) she used to drink her self into a stupor every night now it is only once a week and not to a stupor
I see this as a plus she maintains that it does not help so much but it it is giving her space to be normal the only thing she wants too know is the side effects other than those that she has experienced such as rash all over and such I believe if she can at least lesser her consumpsion of alchohol she will get on top of things if any one has a comment please send it to me sweetmarym@hotmail.com I would appriciate any comments to help as I cannot find web sights for side effects of campral ect yours mary

For those asking whether it's available in the USA it appears at this point it isn't. So I don't know how you go about getting it.As boyreject says, it only works if you want it to work (obviously most of us do want it to work). I found it works for me, it definitely reduces any craving, so I don't physically crave alcohol at all, but there is still a psychological feeling of wanting to but that is easier to cope with, counselling is important or diversionary activity. I'm pretty happy cause I keep myself busy. It is more of an aid than a 'cure' (one day maybe that will happen, but not yet). For those in Australia (like myself) you get it from your GP, as a prescription, it's on medical benefits, so it costs around $21 a month. Seems to be working for me, and good luck to everyone. Craig

G'day people I have a friend who has just taken his first Campral tablets here is hoping that with counselling and the right friends he can again live a life of freedom without the day to day requirement of something to drink to take the edge off. He has stated that he is tired of waking up feeling hungover each day and just wasting each day away.yes to those who have asked is it available in Australia. But only through GP's

I have just been prescribed the drug....haven't started yet coz I have various parties and dinners to attend over the next few days...but when is a good time to stop drinking??? There will always be an occassion we need to attend where alcohol will be...The first day I had the Campral in my hand I did not drink...somehow just knowing I was being helped and listened to by my Doc and concellor was enough...but I have been drinking since. I only drink in the evenings...but when I do, I go on and on and on...Like until 5.00am... 2 to 3 bottles of wine...maybe not that much compared to some but enough to effect my health...I have terrible stomach pains...reflux...and social problems too. I am planning to start taking Campral tomorrow...but is that not wot we always say...tommorow??? Hope I can do it with the aid of Campral??? Anyone in the same boat as me???

I have been prescribed Campral as an aid to a cannabis addiction, as the use of anti D's (ie Xanax) proved to be ineffective. Has there been any study done on the use of Campral with marijuana smokers ?

huh??? to STOP smoking joints you were prescribed xanax as a countermeasure?? i'm sorry, but that sounds totally ludicrous. where on earth is the use of that?? you actually substitute something natural for a chemical? i stopped all downers and antidepressives when i realized that they were messing me up more than anything. and i'm very happy for that. when will the world realize that there are more benefits to be had from marijuna then these state-sanctioned chemicals? but on the issue of campral: i've taken it before and i was actually able to drink whilst taking the pills, but only 2-3 glasses of wine, not more (or rather i didn't want to drink more). is that normal? or maybe i wasn't taking the proper dosage.sila

I have a dual diagnosis client who reports that Campral relieves her also of her addiction to gambling. Im a Psychologist working with gambling addictions and very few practitioners seem to acknowledge this propblem.

I recently, found into a magazine the new drug that helps t the alcholics to reduce the stimulation for not have the desire of drinking any more with the new 'CAMPRAL', and as fast i can i getit from a friend who has a son in germany and proced to order some of the medication and today i fortantuly i have it in my hands, and i'm very exited because if this medication work as you sed , it is going to be a great help for a lot of peapole that have the drinking disese and i'm very happy to can make a test into myself to realy know if it it work as you sed and i'm sure if it made a great help to millons of men women boys and girls please let mee introduce it into the mexican market

My presciption is for 666g ACAMPROSATE x 3 ( divided doses ). I have been on it for 18 months in conjunction with 400mg DISULFIRM daily.At first I was under the misguided impression I was on a placebo. My alcohol addiction spans over 10 years in one form or another. Many de-toxies I can now go to bars etc without the slightest thought for the need for a drink. I must say that I am worried about comming off both drugs. It is better to be sober and taking a few pills than what was my pityfull life as before. Allan

After a bout of pancraitis and a 2 week stay in hospital in severe pain with a 5cm cyst on my pancreas and 3 hourly pain killers. I started Campral and after 2 weeks the craving has diminished and after being pathetically thin for my height have gained weight. This has been my 5th attack of pancreatis. So far it is working...I might add it can be a mind game as well...I do not go to AA but seek personal help thru my GP and a Psycologist.

I don't know if Campral works. It made no difference to me whatsoever. The tablets are still sitting in my cupboard. Perhaps I'll give it another go...... Is it supposed to make you sick if you drink or just take the edge off the craving? I find that just a change in routine is the most effective way. I stopped for 6 months in 99 and wish I could do so again.Good luck everyone

Is this drug available in Australia? I have a student in his 40's who is a severe alcoholic. He is also dependent on analgesics because of a severe sciatica. He has only attempted detox once and this only lasted a few days before he released himself.To be suitable for Campral, does he have to go through detox? A quick note on campral...They only work if you want them to work. You need to have ongoing counselling to make them a success. So don't rely on them being an instant cure! Get the whole package!

having tried campral for the first time ,i thought it was going to be a miraacle for me and my family. sure it was great for a couple of weeks but went back onto the grog. you really need to want to give the piss away ,i dont want the AA stuff anymore, screws your head. its not a miracle tablet to fix your life , but i realise that if your not prepared to make the change it wont work. keep fighting the battle thanks mark.
